               राष्ट्रीय स्वास्थ्य मिशन अन्तर्गत आर.ओ. पी. वर्ष 2022-24 मे स्वीकृत मानव संसाधन के आधार पर, मुख्य चिकित्सा एवं स्वास्थ्य अधिकारी के अधीनस्थ कोरिया जिले में रिक्त संविदा पदों कमांक 01 से 32 तक एंव एड्स नियंत्रण कार्यक्रम अर्न्तगत पद कं0 33 से 35 तक के पदों में नियुक्ति किया जावेगा। राष्ट्रीय स्वास्थ्य मिशन एवं एड्स नियंत्रण कार्यक्रम अर्न्तगत जिला स्तर पर रिक्त संविदा पदों की भर्ती किये जाने हेतु इच्छुक आवेदक नीचे दिये आवेदन प्रारूप (अ) अनुसार दिनॉक ..1.3/08/2024 से दिनॉक 03/09/2024 को संध्या 05.30 बजे तक कार्यालय, मुख्य चिकित्सा एवं स्वास्थ्य अधिकारी, बैकुन्ठपुर, जिला कोरिया, छ०ग० के पते में केवल पंजीकृत डाक/ स्पीड पोस्ट / रजिस्ट्री के माध्यम से ही भेजें अन्य माध्यमों से भेजे गये आवेदन स्वीकर नही होंगे। उक्त रिक्त पदों का विवरण निम्नानुसार है

6. सभी पद पूर्णतः संविदा आधारित है, जो कि सरल कमांक 01 से 32 तक राष्ट्रीय स्वास्थ्य मिशन छत्तीसगढ़ द्वारा जारी मानव संसाधन नीति 2018 के अधीन है तथा सरल क्रमांक 33 से 35 तक एड्स नियंत्रण समिति के नियमानुसार संविदा भर्ती के संदर्भ में आरक्षण व अन्य नियम - शर्ते लागू होंगे। 

। 

7. आवेदक आवेदन प्रस्तुत करते समय आवेदन मे पदनाम, कार्यक्रम का नाम एवं वर्ग (अनारक्षित / अन्य पिछड़ा वर्ग / अनुसूचित जाति / अनुसूचित जनजाति) का नाम अनिवार्य रूप से उल्लेख करें । 

8. आवेदन निर्धारित प्रारूप मे पूर्णतः भरकर बंद लिफाफे में कार्यालय मुख्य चिकित्सा एवं स्वास्थ्य अधिकारी, जिला कोरिया के पते पर पंजीकृत डाक / स्पीड पोस्ट / रजिस्ट्री के माध्यम से ही स्वीकार किये जायेगें । अन्य किसी माध्यम से या सीधे आवेदन स्वीकार नही किया जावेगा । 

9. विज्ञापन जारी होने के उपरांत निर्धारित तिथि एवं समय तक आवेदन प्राप्त ना होने पर या अन्य किसी कारणवश विलंब या डाक द्वारा विलंब के लिये विभाग जिम्मेदार नही होगा। उक्त संबंध मे आवेदन स्वीकार / विचार नही किया जावेगा ना ही कोई अभ्यावेदन स्वीकार किया जावेगा । 

10. आवेदन पत्र निर्धारित प्रारूप (अ) मे पूर्ण रूप से भरे, स्पष्ट एवं पूर्ण आवेदन पत्र ही स्वीकार किये जायेंगे । अस्पष्ट, अपूर्ण भरे आवेदन पत्र अस्वीकार कर दिये जायेंगे । 

11. शैक्षणिक योग्यता जिस संस्था से प्राप्त है, उस संस्था का आवश्यकतानुसार यू.जी.सी., ए.आई.सी.टी.आई. एवं 

संबंधित काउंसिल से मान्यता प्राप्त होना अनिवार्य है। इसके अतिरिक्त अन्य संस्था अमान्य है (इस हेतु वेबसाईट https://www.ugc.ac.in का अवलोकन करें ) । 

चयन प्रक्रिया निम्नानुसार होगी :- 

11.1 अनिवार्य शैक्षणिक योग्यता ( स्नातक / स्नातकोत्तर जो भी अनिवार्य हो उसके सभी वर्षो के कुल प्राप्तांकों के के आधार पर) की गणना अनिवार्य शिक्षा की पूर्ण अवधि मे अर्जित कुल प्राप्तांकों के प्रतिशत के आधार पर पदनाम वार दिए गए सारिणी अनुसार किया जायेगा ।

1. विज्ञापन जारी होने की तिथि मे समस्त शैक्षणिक एंव अन्य अर्हताएं पूर्ण होना अनिवार्य है, अन्यथा आवेदन पत्र तत्काल अस्वीकृत किया जावेगा । इस संबंध मे कोई पत्र व्यवहार मान्य नही होगा। किसी भी परिस्थिति में आवेदन हेतु अंतिम तिथि के पश्चात कोई भी प्रमाण पत्र स्वीकार नही किया जावेगा । अतः अर्हता को सिद्ध करने वाले समस्त प्रमाण पत्र आवेदन के साथ संलग्न करना सुनिश्चित करें अन्यथा आवेदन पर विचार नही किया जावेगा ।

1. मात्र आवेदन करने पर ही किसी का चयन सुनिश्चित नहीं होगा। अंतिम चयन सूची जारी करने के पूर्व मूल प्रमाण पत्रों / दस्तावेजों की जाँच की जाएगी, इसमें सही पाये जाने पर ही उनके चयन हेतु अग्रिम कार्यवाही की जावेगी | 

2. संविदा मानदेयः– संविदा सेवा अवधि के दौरान प्रतिमाह एकमुश्त मानदेय व अन्य राशि प्रावधानित आर.ओ. पी. अनुसार देय होगा, इसके अतिरिक्त किसी अन्य प्रकार का भत्ता / सुविधा देय नहीं होगा । 

3. छ.ग. राज्य एड्स नियंत्रण समिति द्वारा जारी नियम एवं शर्तें ICTC / STI एवं OST सेंटर के लिये विज्ञापित पद कमांक 33,34 एवं 35 हेतु लागू होंगी । 

4. विशेष आरक्षण अर्हता रखने वाले ( विकलांग / भूतपूर्वसैनिक / महिला आरक्षण नियमानुसार होगी ।) अभ्यर्थियों को सक्षम अधिकारी द्वारा जारी प्रमाणपत्र आवेदन पत्र के साथ संलग्न किया जाना अनिवार्य होगा । अन्यथा ऐसे अभ्यर्थियों को आवेदन पत्र में प्रमाण पत्र संलग्न न होने की स्थिति में सामान्य श्रेणी में सम्मिलित किया जावेगा । 

5. अनुसूचित जाति / अनुसूचित जनजाति / अन्य पिछड़ा वर्ग श्रेणी के अभ्यर्थियों को स्थाई जाति प्रमाण पत्र प्रस्तुत करना होगा । 

6. एक अभ्यर्थी द्वारा एक से अधिक पदों हेतु आवेदन करने पर पृथक-पृथक आवेदन पत्र प्रस्तुत करना तथा आवेदन शुल्क पृथक-पृथक जमा करना अनिवार्य होगा । अन्यथा ऐसे अभ्यर्थियों को अपात्र की श्रेणी में रखा जावेगा । 

7. विज्ञापन में विज्ञापित पद अनुरूप निर्धारित अर्हताधारी अभ्यर्थी के ही आवेदन स्वीकार किये जावेंगे।

9. विज्ञापित पदों की संख्या में नवीन एवं बैकलॉग पद शामिल हैं, पदों की संख्या में आवश्यकतानुसार परिवर्तन हो

10. दिव्यांग आवेदकों को चिकित्सा प्राधिकारी, (चिकित्सा प्राधिकारी, जो सरकारी सेवक नहीं है, के द्वारा जारी प्रमाण-पत्र की दशा में प्रारुप चार मे मुख्य चिकित्सा अधिकारी / चिकित्सा अधीक्षक / सरकारी अस्पताल के प्रधान का प्रतिहस्ताक्षर और मोहर) जिला मेडिकल बोर्ड से प्राप्त स्थाई जीवित प्रमाण पत्र की स्वप्रमाणित छायाप्रति संलग्न करना अनिवार्य होगा, केवल ऐसे व्यक्ति सेवाओं / पदों में आरक्षण के लिए पात्र होंगे जो कम से कम 40 प्रतिशत संगत निःशक्तता से ग्रस्त हों । 

11. विशेष सचिव, छत्तीसगढ़ शासन, सामान्य प्रशासन विभाग के पत्र क्रमांक / एफ 20-4 / 2014 / आ. प्र. / 1-3 नया रायपुर दिनांक 27.09.2014 के अनुसार निःशक्तता से बाधित व्यक्तियों के लिए आरक्षण संबंधी नियमानुसार तृतीय श्रेणी के पद में ए.एन.एम. के पदों हेतु OL, OA तथा स्टॉफ नर्स पद हेतु OL निःशक्तता का प्रकार निर्धारित किया गया है। (OL- One leg, OA - one Arm) इन्हीं अभ्यर्थियों में से चयन किया जायेगा ।

6. All the posts are completely contract based, which is under the Human Resource Policy 2018 issued by the National Health Mission Chhattisgarh from serial number 01 to 32 and as per the rules of AIDS Control Committee from serial number 33 to 35, reservation and other provisions regarding contract recruitment are given. 7. While submitting the application, the applicant must mention the designation, name of the program and name of the category (Unreserved/Other Backward Class/Scheduled Caste/Scheduled Tribe) in the application. 

8. Applications completely filled in the prescribed format will be accepted in a sealed envelope through registered post / speed post / registry addressed to the office of the Chief Medical and Health Officer, District Korea. Applications through any other means or directly will not be accepted. 

9. After the issue of advertisement, the department will not be responsible for the delay or postal delay if the application is not received by the prescribed date and time or due to any other reason.
